Make a referral

We accept referrals from patients, carers, medical and social care professionals, and those in need of bereavement support via our online referral forms.

Anyone over the age of 16 with complex problems related to an advanced, progressive, life-limiting or incurable illness can be referred to us for medical support.

Adults who are struggling to cope with grief after the death of a loved one can also be referred to us for bereavement support. No prior connection to St Helena services is required to access our bereavement support.

What type of referral would you like to make?

We advise using Google Chrome or Microsoft Edge when using the online referral form, instead of Internet Explorer.
