Hospice in the Home
Our Hospice in the Home team provide a range of services to support patients and families in their own homes.
More than 90% of the patients and families we care for are supported in their place of residence, such as their own home or a care home or nursing home.
We help our patients to make their own choices and live with dignity by visiting and supporting them and their families at home.
Our Hospice in the Home team includes a number of different services offering individual care and total support to local people living with incurable illness. These services exist to complement, not replace other local health and social care providers, such as GPs, community nurses and social services – and we work very closely with our colleagues at these providers to ensure the best possible care is provided to those in need.
Hospice in the Home includes:
SinglePoint is our 24/7 advice and support line and care coordination hub. Our SinglePoint team can make an assessment over the phone or visit if necessary, and help people access the support they need when they need it. Find out more about SinglePoint.
Community clinical nurse specialists (CNS) or other community services can visit a patient at home to provide support when pain or symptoms are proving difficult to control or if there are complex needs which cannot be managed by other healthcare professionals such as GPs or District Nurses.
Counsellors, family support, physiotherapists, occupational therapists and therapy assistants may also visit to provide additional emotional, practical and spiritual advice and support to help people stay as independent as possible and to improve quality of life. This includes advice on managing getting around and day to day activities.
Specialist Palliative Care Virtual Ward aims to manage patients needing more intense input who would otherwise need admission to the Hospice or Hospital. These patients are overseen by the doctors in Specialist Palliative Medicine.
We help people plan for future care, including the sort of care they might like and where they may want to be cared for.
Being able to be open and share these views with family and friends and with the professionals involved in an individual’s care can make staying at home more possible if that is the patient’s wish. We use the My Care Choices Register to achieve this.
